Botox vs. Fillers: What's the Difference and Which One Do You Need?

I can't tell you how often someone comes into Aesthetic Touch by Raya and says, "I want Botox in my lips" or "Can I get filler for my forehead wrinkles?" And every time, I'm glad they asked — because Botox and fillers are two very different things, and mixing them up is super common.

So let's break it down in plain English. No medical jargon, no confusing terminology. Just a straightforward explanation of what each one does and how to figure out which one (or both) you actually need.

The Simplest Way to Think About It

Botox freezes muscles (gently). It stops certain muscles from contracting, which smooths out the wrinkles those muscles create.

Fillers fill things in. They add volume where you've lost it or enhance areas you want to be fuller.

One targets movement. The other targets volume. That's basically the whole thing.

When You Need Botox

Botox is your go-to when wrinkles are caused by muscle movement — also called "dynamic wrinkles." These are the lines that show up when you're making an expression and then disappear (or mostly disappear) when your face is relaxed.

The classic Botox zones:

  • Forehead: Those horizontal lines when you raise your eyebrows? That's your frontalis muscle working. Botox relaxes it.

  • Between the brows: The "11" lines that make you look angry or stressed. Probably the most popular Botox area.

  • Crow's feet: The lines that fan out from the corners of your eyes when you smile. A little Botox here goes a long way.

  • Bunny lines: Those crinkles on the sides of your nose when you scrunch up your face.

The common thread: all of these are caused by repetitive muscle movement over time. Botox addresses the movement, and the wrinkles soften as a result.

When You Need Fillers

Fillers are your go-to for volume loss, structural changes, or enhancement. These address "static" concerns — things that are visible even when your face is completely at rest.

Common filler areas:

  • Cheeks: Volume loss in the cheeks is one of the first signs of aging. Filler restores that fullness and creates a subtle lift.

  • Lips: Whether you want more volume, better shape, or just to restore what you had in your 20s.

  • Nasolabial folds: The lines that run from your nose to the corners of your mouth. These deepen as cheek volume drops.

  • Marionette lines: The lines from the corners of your mouth down to your chin. These can make you look sad even when you're fine.

  • Under-eyes: Those dark hollows that make you look exhausted no matter how much sleep you get.

  • Jawline and chin: For definition, contour, or balancing your profile.

The common thread: all of these are about adding or restoring volume, not relaxing muscles.

The Confusing Part: When You Need Both

Here's where it gets interesting. A lot of the time, people don't need just Botox or just filler. They need both.

Real example: someone comes in bothered by deep forehead lines. I look at them and see that yes, they have dynamic forehead wrinkles (Botox territory), but they also have volume loss in the temples and upper face that's making the forehead look more hollow and making the wrinkles more pronounced (filler territory).

If I only did Botox, the wrinkles would soften but the area would still look hollow. If I only did filler, the volume would improve but the wrinkles would still crease when they raise their eyebrows. But together? Chef's kiss.

Other common Botox + filler combos:

  • Botox in the forehead + filler in the cheeks for overall facial rejuvenation

  • Botox in the crow's feet + filler under the eyes for a well-rested look

  • Botox between the brows + filler in the lips for a total refresh

Stuff I Hear All the Time That Isn't True

"Botox and filler are the same thing." They're not. At all. Different products, different purposes, different mechanisms.

"I'm too young for Botox." If you're seeing lines when you make expressions and they're starting to stick around at rest, Botox can help regardless of your age. Plenty of people in their late 20s use it preventatively. There's no minimum age for wanting to look good.

"Fillers are permanent." Nope. The most common fillers are hyaluronic acid-based and are gradually absorbed by your body. They can also be dissolved if needed.

"I should just pick one." You don't have to. A lot of my best results come from combining the two strategically. It's not about choosing sides.

So Which One Do You Need?
