How Long Does Botox Last? An Honest Answer from a Plano Injector

I get this question every single day. And honestly, I love that people ask it upfront because it means they're thinking about Botox as an investment — which it is.
So let's talk about how long Botox actually lasts, what makes it wear off faster for some people, and how to get the most mileage out of each treatment.
The Short Answer
For most people: 3 to 4 months. That's the window where your results will look their best.
But if I left it at that, this would be a pretty boring blog post. So let's dig in.
The Real Timeline
Here's what happens after you get Botox, week by week:
The first few days: Not much. You might feel a slight heaviness in the treated area, but you won't see a visible change yet. This is normal. The Botox hasn't fully kicked in.
Around day 5: Things start happening. You'll notice the muscles beginning to relax and the lines starting to soften. You might catch yourself trying to raise your eyebrows and realizing it takes a little more effort than usual.
Days 10 to 14: This is the sweet spot. Full results are in. The lines are smooth, the muscles are relaxed, and you're looking at the finished product. This is when I love hearing from clients because they're usually thrilled.
Months 1 to 3: You're cruising. Results are holding steady and you're probably getting compliments about looking well-rested.
Month 3 to 4: The Botox starts to wear off. Movement gradually comes back, and you might notice faint lines reappearing. This is totally normal — the product has run its course and it's time to think about your next appointment.
Why It Lasts Longer for Some People
Here's the annoying truth: two people can get the exact same Botox treatment and have it last different amounts of time. Bodies are just different. But there are some patterns.
Metabolism matters. If you have a fast metabolism — you're very active, you exercise a lot, you just burn through things quickly — you'll likely process Botox faster. I've noticed that my clients who are serious athletes or gym regulars tend to come back closer to the 3-month mark.
The area matters. Forehead Botox tends to wear off pretty consistently at 3 to 4 months. Frown lines often hold a bit longer — sometimes up to 5 months — because those muscles are smaller. Crow's feet tend to be closer to 3 months because we use those muscles constantly (every time you smile, squint, or look at your phone in the sun).
Consistency matters the most. This is the part I really want you to hear. When you keep up with regular Botox treatments, each session builds on the last. The muscles gradually get weaker from not being fully used. Over time, I've seen clients go from needing treatments every 3 months to every 4, even 5 months. It's a real thing.
But if you get Botox once, let it completely wear off, wait a few months, and then come back — you're essentially starting from scratch every time. It still works, but you miss out on that cumulative benefit.
Things That Make Botox Wear Off Faster
A few things work against you:
Intense exercise. I'm not saying don't work out (please keep working out). But very intense, high-frequency exercise can speed up how quickly your body metabolizes Botox. It's not dramatic enough to skip the gym over, but it's worth knowing.
Sun exposure. UV damage is bad for your skin in every way, and it can make wrinkles more prominent as your Botox wears off. Sunscreen is your best friend. Wear it every day. Yes, even when it's cloudy.
Stress. Stress causes you to tense your facial muscles more — furrowing your brow, clenching your jaw, squinting. All that extra muscle activity can work through Botox faster.
Not enough product. Sometimes people want to save money by getting a lower dose. I understand the impulse, but under-dosing Botox usually means it wears off faster, which means you need to come back sooner, which means you end up spending more in the long run. Getting the right dose from the start is actually the more cost-effective approach.
How to Make It Last Longer
You can't completely control your Botox longevity, but there are things that help:
Stay consistent. Book your next appointment before your Botox fully wears off. Treating the muscles while they're still partially relaxed is the best way to build long-term results.
Wear SPF daily. I cannot stress this enough. SPF 30 or higher, every single day. It protects your skin, preserves your results, and prevents future wrinkles. It's the cheapest anti-aging treatment on the planet.
Have a basic skincare routine. You don't need 15 products. Just a good retinol or vitamin C serum, a moisturizer, and sunscreen. These keep your skin healthy between appointments and help your results look better.
Don't touch your face after treatment. For the first 24 hours, leave the treated area alone. No rubbing, no pressing, no picking. Let the Botox settle where it was placed.
What If I Stop Getting Botox?
Nothing bad happens. Seriously. Your face just gradually goes back to how it looked before you started. Botox doesn't make your wrinkles worse if you stop — that's a myth. You just return to your natural baseline.
Some of my clients get Botox for a few years, take a break, and come back whenever they feel like it. There's zero obligation to continue, and there's no downside to taking time off.
